The GUIDE (Guiding an Improved Dementia Experience) program connects you with a care navigator who can help manage the complexities of dementia care. Join our webinar on Thursday, May 28 to learn how we can help you navigate this journey.
“I feel lucky to have this support as it has become increasingly difficult for Dad to accept guidance from me alone. Having professionals reinforce safety recommendations is incredibly helpful as we navigate this transition.”
We support families with expert dementia care navigation designed to prevent hospital visits and keep your loved ones healthy at home. You don’t have to do this alone.
What the GUIDE Program provides:
Lean on one point of contact for help managing appointments and medical, emotional and daily living needs.
Help connecting to vital services like medical equipment, prescriptions, therapy, meals, and transportation.
Connect with a trusted member of your care team for timely help and guidance.
Tap into training, resources and peer support for connection and up to $2,500 in coverage for paid caregiving.
Register for the webinar for a live Q&A, or email GUIDE@bluestonemd.com.
