Dr. Todd Stivland is the founder and CEO of Bluestone Physician Services. With more than 25 years’ experience as a family physician, Dr. Stivland is dedicated to changing the way health care is delivered.
From its founding in 2006, Bluestone grew from a small start-up to the largest on-site provider for assisted living communities in the country. Dr. Stivland is active in state and national health care reform and has served on the Minnesota governor’s health care finance task force.
In his spare time, you can find Dr. Stivland spending time with his family, traveling, fishing or, ideally, anything that combines all three.

Sarah Keenan, R.N.
Chief Clinical Officer
President, Bluestone Integrated Care
Sarah Keenan has worked with seniors in the health care industry for more than 25 years, providing community care expertise and implementing clinical programs, care management, and care coordination programs.
Sarah has been with Bluestone Physician Services since 2011, with oversight over clinical services, including care coordination and care management. Sarah’s background is in long-term care and managed care government programs.
In her spare time, Sarah enjoys boating and biking in the MN summers. She does her best to enjoy MN winters but most often can be found by a fire.

Tim Koehler, CPA
Chief Operating Officer
President, Bluestone National, LLC
Tim Koehler is COO of Bluestone Physician Services and President of Bluestone National. He is dedicated to expanding the Bluestone Physician Services model in the national health care market, improving the way medical care is delivered to vulnerable and high-risk patient populations. Prior to joining Bluestone, Tim spent 18-plus years at UnitedHealth Group, including roles in executive leadership, operations, finance, sales, account management, acquisitions integration, new market strategy, personnel mentoring and development. He was one of the founders and president of the Diabetes Prevention and Control Alliance, a UnitedHealth Group organization.
In his spare time Tim enjoys: traveling, skiing, fishing, woodworking, college football, watching his kid’s sports, and any other outdoor activity he can share with his family.

David Nelsen, CPA
Chief Financial Officer
David Nelsen joined Bluestone Physician Services as CFO in 2017. Prior to Bluestone, he worked at American Dental Partners for 16 years, overseeing all financial operations and supporting the growth of the organization through both mergers and acquisitions activity and organic growth. He also held a variety of finance positions at The Pillsbury Company and was an audit manager with KPMG.
In his spare time, David enjoys traveling, running, hiking and watching college basketball.

Vice President of Clinical Operations
Candice Levy has been with Bluestone Physician Services since 2009, where she serves as VP of Clinical Operations and Bluestone’s Compliance Officer. Her role includes developing and administering the organization’s electronic health record, training programs, regulatory compliance & enterprise risk management, process improvement initiatives, and program development.
Her career in the healthcare industry has focused on improving software and technology’s usability to streamline data and workflows for healthcare providers and support staff. She has played a role nationally in healthcare interoperability and how healthcare systems share data through implementing and improving health information exchanges.
In her spare time, Candice enjoys spending time outdoors, paddle boarding, camping, and motorsports activities.

Dennis O’Hare, M.D.
Vice President of Medical Affairs
Dr. Dennis O’Hare joined Bluestone Physician Services in 2020 as Vice President of Medical Affairs. He is double-boarded in Family Medicine and Geriatric Medicine and has always maintained a clinical practice as a leader, believing what he brings to the decision table is the voice of the patient and the voice of the clinician. He began his career as an owner in River Valley Clinic, a Twin Cities multi-site group practice that became one of the nidus groups of the Allina Health clinic system. He was a founder of one of the first Hospitalist services in the country at United Hospital in Saint Paul.
As a clinically active executive leader he brings significant expertise in driving quality, safety and clinical operations. He has held leadership positions in hospitals, ambulatory clinics, systems and health plans. He is also active in leadership in the MN Medical Association and the American Medical Association.
In his spare time he enjoys family, friend and church activities; cabin life and projects; and trying to sample most IPAs.

Brian Holsten, MS, SHRM-SCP
Vice President of Human Resources
Brian Holsten possesses 30+ years of Human Resources experience across a variety of industries, including hi-tech, med-device, pharmaceutical, financial services and insurance.
Prior to joining Bluestone Physician Services in 2020, Brian was the VP of Human Resources for Perforce Software, Inc., where he led the global HR function for this enterprise software company. This included oversight of talent acquisition, total rewards, employee relations, M&A, HR technology and global payroll.
Brian has also used his extensive HR experience to teach undergraduate courses as an adjunct professor at Bethel University in St. Paul, MN.
In his spare time, Brian enjoys running, downhill skiing and a variety of other wellness activities.

Jessie Waks, APRN, CNP, GNP-BC
Vice President of Clinical Practice
Jessie joined Bluestone Physician Services as a Nurse Practitioner in 2009. Her experience in Critical Care and Hospice drew her to Bluestone’s innovative care model, where she could serve complex and vulnerable patients in the community setting.
Now with Bluestone for over a decade, Jessie has played an integral role in the expansion of Bluestone’s model of care. Provider on-boarding, advancement and education are a few of the many programs she has spearheaded in her tenure. Through her leadership, Bluestone has trained over 125 Nurse Practitioner and Physician Assistant students in conjunction with local universities. Her passion for quality patient care and provider excellence is clear to everyone, both in and out of Bluestone.
In her spare time, Jessie can be found doing anything (especially active) with her daughters, running up mountains, or traveling to anywhere Jack Johnson is performing.
