Bluestone GUIDE Program
Improving the dementia care experience for patients and caregivers
When dementia becomes part of your family’s story, it can feel isolating and overwhelming to navigate. You’re not alone. Bluestone is a trusted partner now offering care navigation through CMS’s Guiding an Improved Dementia Experience (GUIDE) Program, a new set of services designed to improve the quality of life for people living with dementia, as well as their caregivers.
Access to supportive care navigation is proven to help caregivers and their families:
- One point of contact to help manage medical, emotional and daily living needs.
- Help prevent avoidable hospital visits and manage medication adjustments.
- 97% of caregivers would recommend collaborative dementia care to another caregiver. (Care Ecosystem | JAMA)
- 20 years of experience caring for the unique challenges of aging and dementia.
- Reduce stress and caregiver burden with resources and peer support for connection and up to $2,500 in coverage for paid caregiving.
How the GUIDE program works
Patients are assigned a dementia-educated Care Navigator who will coordinate and facilitate access to services, educate about the dementia diagnosis and best practices for caregivers, and provide support that people living with dementia need to remain in their homes safely.

*Patients who have Traditional Medicare and a diagnosis of dementia are eligible for GUIDE services at no cost to them. Your Medicare benefits will NOT change. You remain eligible to receive the same Medicare benefits, and you still have the right to use any doctor, other health care professional, or hospital that accepts Medicare, at any time. Who does
Bluestone serve?
Bluestone GUIDE Dementia Care Navigation services are offered at no cost to eligible patients.
Who is eligible?
- Individuals who reside in assisted living, memory care, and independent living communities that currently offer Bluestone primary care.
- Patients who have Traditional Medicare and a diagnosis of dementia or another neurocognitive condition.
Important Notes:
- Patients are not required to be a primary care patient with Bluestone to participate.
- The program will be rolled out to eligible facilities throughout 2026.
Who is not eligible?
- Individuals who are on hospice.
- Individuals who are still living at home (outside of eligible community types).
