Bluestone GUIDE Program

Improving the dementia care experience for patients and caregivers

Medicare selected Bluestone to participate in a new pilot program called the Guiding an Improved Dementia Experience (GUIDE) Model, which is designed to improve the quality of life for people living with dementia, as well as their caregivers who give them support with routine tasks.

The Bluestone GUIDE program will begin in July 2025 — available at no cost to eligible patients* (those who have Traditional Medicare and have a dementia or other neurocognitive diagnosis).


How the GUIDE program works

Patients are assigned a dementia-educated Care Navigator who will coordinate and facilitate access to services, educate about the dementia diagnosis and best practices for caregivers, and provide support that people living with dementia need to remain in their homes safely.

Comprehensive assessment and care plan

Care Navigators will identify individual health needs and create a tailored plan to provide clinical and non-clinical services.

Coordination

Care Navigators will host regular check-ins to address medical and behavioral needs, and provide support during care transitions to facilitate clear communication between patients’ care team, community staff, and family.

Caregiver support and education

Care Navigators will share best practices for dementia care, resources to alleviate the stress that comes with caregiving, and 24/7 access to a support line.

How to enroll

Bluestone will reach out to eligible patients and/or their designated healthcare power of attorney with information on the enrollment process in June, with services beginning in July. 

If you are interested in GUIDE services but Bluestone is not currently servicing your assisted living community, please contact GUIDE@bluestonemd.com so we can collect your information and interest — we plan to bring GUIDE services to new areas in early 2026. 

*Patients who have Traditional Medicare and a diagnosis of dementia are eligible for GUIDE services at no cost to them. Your Medicare benefits will NOT change. You remain eligible to receive the same Medicare benefits, and you still have the right to use any doctor, other health care professional, or hospital that accepts Medicare, at any time.
