“My wife was strong and independent. As her Parkinson’s progressed, wheelchair transfers became difficult when she’d resist help. The Bluestone team spent time observing and recommended a gentler, more effective approach to our community’s nursing team. Having that thoughtful teamwork was a true luxury.”

How do I know if behavioral health support could help me or my family member?

If you’re interested, ask your provider at any visit. Your provider may recommend behavioral health care management if they notice you’re:

  • Feeling isolated, overlooked, or having trouble adjusting after a move.
  • Experiencing frequent hospitalizations.
  • Living with symptoms of depression, anxiety, delusions or hallucinations.
  • Showing aggression, social withdrawal or avoidance.
  • Struggling with substance use, eating disorders, self-harm, or suicidal thoughts.
Is there an extra cost?

We bill your insurance each month. Medicare covers Behavioral Health Integration and depending on your insurance plan, you may have a copay or deductible. In 2025, 1 in 10 patients had an out-of-pocket cost after meeting their deductible. Patients with Original Medicare typically pay about $15–$35 per month (around $23 on average). Patients with Medicare Advantage, secondary insurance, or Medicaid typically pay a range from $0–$50 per month (about $18 on average).

Is this the same as therapy?

No. Behavioral health care management is practical support integrated into your medical care, not clinical therapy. We focus on coaching and daily tools — like journaling and breathing — to help you manage symptoms and triggers alongside your primary care provider.

Who will I meet with and how often?

You will meet with a behavioral health care manager once a month, who also works closely with your primary care provider and a psychiatric consultant to support your care as a team.

Do you offer psychiatric services?

With Behavioral Health Care Management, your primary care provider collaborates with a psychiatric consultant and behavioral health care manager to guide treatment, adjust medications when needed and support your emotional well-being — without extra appointments.

What support does a behavioral health care manager offer?

Your behavioral health care manager provides calm, practical support using tools like goal setting, guided conversations, and breathing exercises.

They help identify environmental triggers and suggest simple changes — like adjusting routines or reducing clutter — to create a calmer setting.

They also partner with your family and community’s team to provide the education, coaching, problem-solving, and resources needed to ease symptoms.
