How do I enroll with Care Coordination?

If you are enrolled in Minnesota Special Needs BasicCare (SNBC), Minnesota Senior Care Plus (MSC+) or Minnesota Senior Health Options (MSHO), your health plan will connect you to support from a Bluestone Care Coordinator — a registered nurse or licensed social worker — who helps you navigate your healthcare and support services.

How does a Care Coordinator help me?

Your care coordinator provides expert guidance on benefits, supports your health goals, helps create a personalized care plan, and connects you to services and community resources. They can help with referrals, prescriptions, durable medical equipment, therapy, meals, transportation, and other supports that help you stay well.

Will my care coordinator work with my existing providers?

Yes. Your care coordinator communicates with your primary care provider, specialists, and other members of your care team to keep everyone aligned.

How often will I hear from my care coordinator?

Your care coordinator will work with you to determine what level of support feels right, based on your needs and preferences.

 

Is there a cost for Care Coordination?

Care coordination is covered by your health plan. Our team can help you understand any costs or coverage details.
