We bill your insurance for our visits just like a standard clinic. Depending on your coverage, co-pays or cost-sharing may apply for other services, including support between visits and behavioral health care.
We will always obtain your consent before enrolling you in any program or service.
Your care team is led by your primary care provider — a physician assistant or nurse practitioner, who is supported by a collaborating physician — who collaborates with a clinical assistant and operations teams working to coordinate follow ups.
Depending on your health needs, you also may access extra support from a behavioral health care manager, complex care manager or dementia care navigator.
Your provider visits your community regularly to proactively manage chronic and complex health needs.
Visits are routinely scheduled based on your health needs with acute visits available if new concerns arise — in person or via telehealth.
During visits, your provider focuses on what matters most to you, including managing medical conditions, reviewing medications, assessing fall risk, memory, mood and overall well-being, and will update your care plan to align with your goals.
We make referrals to and coordinate with specialists or other care services when needed. You have 24/7 access to your care team for urgent medical concerns by phone or through Bluestone Bridge, so support is always within reach.

When questions arise, our team responds fast — urgent medical concerns within one hour, 24/7, and routine messages within four hours during clinic hours or next business day after hours.
*To protect your privacy, we need your permission before sharing health information. If you have a Medical Power of Attorney, Health Care Directive, or guardianship paperwork that names who can access your information, we’ll follow that guidance.
If this isn’t already outlined, your provider will talk with you at your first visit about who you’d like involved in your care and help document your permission. Once that’s set, our team will contact them to help get registered on the Bridge. Bridge support is available via phone: 855-794-9476
