What is an Accountable Care Organization (ACO)?

An ACO is a group of healthcare providers that work together to improve the quality and experience of care for people who have Original Medicare.

ACOs are designed to put patients at the center of their care and help them navigate a complex health system.

Bluestone ACO allows us to provide extra help to patients with Original Medicare. We assign them a Complex Care Manager who proactively focuses on delivering the right care at the right time, while avoiding unnecessary services.

How do I join the Bluestone ACO?

If you receive your primary care from us and have Original Medicare, you may already be eligible to participate.

In most cases, enrollment happens automatically based on your primary care relationship with us — there’s no extra paperwork, cost or change to your Medicare benefits.

You keep full choice of Medicare providers while gaining access to additional care coordination and support.
