If you have Traditional Medicare, you can be part of the Bluestone Accountable Care Organization and receive extra support to help you stay connected to the care you need.
Complex care managers offer extra support for your day-to-day needs — they learn your health history and step in early when symptoms change to help you stay healthy at home.
We monitor and help address emerging concerns at home — coordinating support, adjusting care plans and keeping you on track.
If you visit the emergency department, we connect with your hospital care team to make sure treatment aligns with your wishes.
We can arrange direct admission to skilled nursing facilities to avoid unnecessary hospital stays and ease stress.
We collaborate with your care team after a hospitalization — equipment, therapy, medications and follow-up visits — to help you recovery safely at home.
“After calling to share my patient’s history, I’ve had Emergency Department doctors say, “I had no idea they had that diagnosis. Now we don’t have to run so many tests; we can just run two because we have this information!”
