The Bluestone Bridge is our secure communication portal that keeps your residents, their families, your staff and other care partners connected to our care team.

Complete this Community Staff & Service Partner Registration Form to register. If you do not have the community or service agency name and code, please contact your Senior Living Relationship Manager for assistance.

Service partners must supply appropriate documentation to support their need for patient access and validate their employment with that service agency. Service Partner (home care and hospice) access is limited to only direct patient facing care team members.

The Bluestone Bridge is our secure communication portal that keeps your residents, their families, your staff and other care partners connected to our care team.

It is also a centralized portal for orders and follow-up to manage labs, medications, imaging and referrals.

Every visit comes with a timely summary for your residents and their registered family members.

And when questions arise, our team responds fast — urgent Bridge messages within one hour, 24/7, and routine messages within four hours during clinic hours (or next business day after hours).
